 An advance directive, sometimes also called a "living will", is a legal document that lets you outline your healthcare preferences in case you’re ever unable to make decisions for yourself. It ensures your medical treatment aligns with your wishes and can also name someone you trust to make decisions on your behalf. It’s a simple way to take control of your future care. 

 Some states require an advance directive to be notarized, while others accept it with the signatures of witnesses. Additionally, some states give you the option to choose between notarization or witnessing.

Discuss the responsibilities of a healthcare proxy and provide tips to help you select the right person for the role.

3. Briefing on Your State’s Requirements

Explain whether your state requires notarization, witnessing, or gives you the option for either, so your document complies with the law.

4. Discussion About Your Healthcare Preferences

Facilitate a conversation about your medical care preferences, ensuring the form reflects your unique values and choices.

5. Personalized Advance Directive Form

After the session, you’ll receive a finalized form customized to your preferences and state regulations, delivered via mail or email.
